LIFETIME HEARING SERVICES' PRIVACY POINTS SUMMARY
 
1)  We may use your medical information to treat you.
2)  We may use your mdeical information to bill and collect payment for
      the services and items you may receive from us.
3)  We may use your medical information to operate our business.
4)  We may use your medical information with "business associates"
      that perform certain services on our behalf (i.e. our accountant).
5)  Unless you object, we may disclose to a family member, a relative, a
      close friend, or any other person that you identify, your medical
      information that directly relates to your visit here.
6)  We may use your medical information in order to communicate with
      you about a product or service related to your treatment.
7)  We may use your medical information in response to a court or
      administrative order, if you are involved in a lawsuit.  We can also
      release your medical information if asked to do so by law enforce-
      ment officials.
8)  Our office may release your medical information for workers' comp-
      ensation and similar programs.
 
You, as our patient, have the right to inspect your medical information, and to amend your medical information if you believe it is incorrect or incomplete ( this must be done in writing ).  You are also entitled to a paper copy or our entire privacy notification act at any time.  Just ask our receptionist, and she will get you one.  A copy is available for your use in our lobby.
